Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

- You request your old GP to send your medical file to us.

We are open for new registrations. In case you live less than 10 minutes by bicylce or on Heijplaat, feel free to subscribe.
Please fill out the registration form carefully.
If you are subscribing with your entire family, make sure to complete a separate form for each family member, using a personal and unique email address for each individual and use a Dutch telephone number.
